What to bring with you to all appointments:
- Your insurance card and any authorization your insurance has sent you.
- List of prescription medications and dosages
- List of allergies
- List of questions you may have
- Recent test results related to your condition
- Paper and pencil to take notes

Sending Radiology Images
The UCSF Department of Radiology Imaging Library offers a secure and convenient way for you to electronically send your images to our clinic via web. If you are interested in sending us your images via ReferralBox, please read this document for more information and then contact the clinic for an email notification to be sent to you.
