How to become New Patient
The first step to become a new patient at the Movement Disorders and Neuromodulation Center is to contact our office at (415)-353-2311 and have your physician submit a referral form

Requesting an Appointment
Be sure to have your current neurologist or primary care physician fax or send your medical records, including MRI films, to us before the date of your visit.

Health Insurance
Be aware that many insurance plans will require an official insurance referral from your primary care physician before you can schedule a visit with a specialist at UCSF. Contact your insurance to learn whether you need to obtain an authorization prior to your visit.
